In the last decade, the possibility of therapeutic treatment of keratoconus became feasible.
It's about the corneal therapy Cross Linking or cross fusion with ultraviolet rays (UltraViolet - KrossLinking - UV-X).
Cross Linking method increases corneal density, stops the progression of keratoconus and allows the avoidance of corneal transplant surgery.

Essence of Kross Linking method
Under the influence of special shaped and dosed ultraviolet rays (UVA) occurs a photochemical reaction of ionization with release of free atomic oxygen. Atomic oxygen causes cross-linking of collagen molecules, which make a corneo-scleral shell, in a single three-dimensional network. This process significantly increases the density of corneal tissue and stops the progression of keratoconus.

Kross Linking steps
Superficial local anesthesia is administered by using droplets. With special instruments, the top layer - epithelium of the cornea is removed.
During 30 minutes, every 2-5 minutes, riboflavin (vitamin B in a solution of dextran T500) is dropped on the cornea.
In the next 30 minutes, dripping riboflavin occurs simultaneously with the corneal laser exposure.
On the patient's eye is fixed a protective contact lens that is removed in about 4-5 days. For a month the patient must drip a special solution.

Previously there was possible only the painful procedure in the postoperative period - Epi-off. The difference between Epi-off and Epi-on in case of Cross Linking is in the removal of the external corneal epithelial layer at the beginning of the procedure.

The advantages of Epi-on Kross Linking:
• The procedure is comfortable for the patient;
• The procedure lasts less;
• There is no pain syndrome;
• It also reduces the chances of infection, because the epithelial layer remains untouched;
• Short period of rehabilitation;
• The patient can return to normal wear of contact lenses already after 48 hours;
• After this procedure does not remain scars on the cornea.

Seiler lamp of last generation - Xlink:

 

• Radiation exposure time is reduced from 30 to 5 minutes, because the intensity of energy flow increase,  which offers the patient comfort and reduce the risk of infections;
• has a much better homogenization of the beam (in the previous model were seven diode, in the current one are 50 diode) which allows:
conduction of uniform irradiation, even in the case of uneven corneas with astigmatism;
reduction of astigmatism after the procedure.

Seiler lamp is equipped with a special homogenizer (IROC company patent), that permitts a uniform flux of ultraviolet energy to the entire surface of the cornea, regardless of unevenness level in different portions. It is a very important factor, because namely the homogeneity beam provides a uniform effect on the cornea, that allows to avoid the occurrence of "hot" and "cold" points (hypo- and hiper reactions).

The special formula of riboflavin serves as a photosensitizer of Kross Linking reaction, but also protects the inner layers of the cornea and the entire ocular globe, since it absorbs over 90% of all radiation on the perimeter of 300 microns of the cornea. Thus, the action of ultraviolet radiation and corneal endothelium does not reach the inner parts of the eye and is much safer for the eyes. As a result of 15 years of clinical experience, was demonstrated the stability and reliability of the Kross Linking method.
